This notice explains, in everyday language, what happens to the details you give us when you fill out a Lyvera inquiry form. We’ve kept it short and direct so you know exactly what to expect.
What we collect
When you submit an inquiry, we may collect:
- Your name.
- Your contact information, such as your phone number and email.
- The type of care you’re exploring and the care needs you describe.
- Your location.
- Your timeline — how soon you’re looking for care.
- Your comfort level with budget.
We only collect what you choose to share on the form. You don’t need to give us anything you’re not comfortable sharing.
Who receives your inquiry
Here’s the most important part: when you submit an inquiry, it will be shared with participating licensed care providers so they can help with the care you’re looking for. Those providers may then contact you directly — by phone, text, or email — to talk about your situation and how they can help.
We share your information to connect you with care. That’s the whole point of submitting an inquiry, and we want you to know it will happen.
Who we are — and who we are not
- Lyvera is free for families. You never pay us to submit an inquiry or to be connected with a provider.
- Lyvera is not a care provider. We don’t deliver care ourselves — we route your inquiry to providers who do.
- Lyvera is not a government agency and is not affiliated with any state program.
Do we sell your information?
No. We do not sell your personal information beyond the matching and sharing with participating providers described in this notice. Sharing your inquiry with providers so they can reach you is the only way your information moves.
How long we keep your information
We keep your inquiry information for as long as it’s useful to help connect you with care and to run our service responsibly. If you no longer want us to keep it, you can ask us to delete it at any time and we will.
How to opt out or request deletion
You’re in control. If you’d like to opt out, stop being contacted through us, or have your information deleted, just email us at hello@lyvera.co and we’ll take care of it. Please keep in mind that if a provider has already reached out to you directly, you may also want to ask that provider to stop contacting you.
